Attention Parents of Sixth Grade Students: Before your child enters seventh grade, your child is required to have the following vaccinations if your child has not had them:1) Chickenpox 2) Three doses of the Hepatitis B vaccine If your child has had chickenpox (the disease), then a note from your child's physician or parent indicating the date of the disease must be returned to the school nurse if you have not already done so. Therefore, if your child is currently in 6th grade please remember to ask your child's physician if your child is up to date when your child has his/her sixth grade physical. For 2010-11 school year, ALL students who have not had the Chicken pox disease will be required two doses of the varicella vaccination. In addition, ALL seventh grade students will be required to have the Tdap vaccination (if at least 5 years since last tetanus and diphtheria), the Meningococcal vaccine, and two doses of varicella by 2010-2011 school year.
Physical Exams are required for students in grades K, 6, and 11. Dental exams are required for students in grades K, 3, and 7. If you choose to take your child to his/her private physician or dentist, instead of your child participating in the free school screening, below are links for your convenience that you can download the forms.
